Our Levels of Care
ROUTINE HOME CARE
This is the most common level of care utilized when symptoms are well maintained. The level of care follows a calendarized visiting schedule with the interdisciplinary team.
CONTINUOUS HOME CARE
Provides a skilled nurse at bedside up to 24 hours a day until symptom management is back under control. This level of care is provided in the patient’s home, assisted living facility, or board & care home.
RESPITE CARE
This is a level of care that hospice provides specifically to support the family/caregivers of patients living in a private residence. Respite provides a break from the responsibilities of being a full-time caregiver
GENERAL INPATIENT CARE (GIP)
Used to address severe symptoms that may need careful monitoring or intervention. Provided only in a skilled nursing facility when symptoms cannot be managed at home or in any other setting, and require 24/7 monitoring with a Registered Nurse (RN).
